دکتر آزاده فرحی

خانم دکتر آزاده فرحی در سال 1340 در شهر بیجار متولد شد.

وی پس از اتمام تحصیلات خود در مقطع دکترای عمومی در دانشگاه علوم پزشکی تهران سال 1367 مدرک تخصص چشم پزشکی خود را از دانشگاه شهید بهشتی در سال 1373 و فوق تخصص اکولوپلاستیک، استرابیسم و اربیت، مجاری اشکی از دانشگاه شهید بهشتی در سال 1378 اخذ کرد.

دکتر آزاده فرحی (چشم پزشک) با گذراندن دوران طرح خدمت پزشکی خود در استان کردستان فعالیت حرفه ای خود را از سال 1373 آغاز کرد و در سال 1379 به بیمارستان فوق تخصصی چشم پزشکی نور پیوست .

دکتر آزاده فرحی (چشم پزشک) خدمات جراحی پلاستیک چشم، پتوز یا افتادگی پلک، مجاری اشکی، اربیت و استرابیسم ،اصلاح عیوب انکساری( پی آر کی، لازک، فمتولیزیک، اسمایل)، کاتاراکت(آب مروارید) را در بیمارستان چشم پزشکی نور انجام می دهد.
