خانم دکتر فدرا حاجی زاده در سال 1348 در شهر تهران متولد شد.
وی پس از اتمام تحصیلات خود در مقطع دکترای عمومی در دانشگاه علوم پزشکی تهران سال 1374 مدرک تخصص چشم پزشکی خود را از دانشگاه علوم پزشکی تهران در سال 1378 و فوق تخصص شبکیه از دانشگاه علوم پزشکی تهران در سال 1382 اخذ کرد.
دکتر فدرا حاجی زاده (چشم پزشک) با گذراندن دوران طرح خدمت پزشکی خود در استان تهران فعالیت حرفه ای خود را از سال 1378 آغاز کرد و در سال 1386 به بیمارستان فوق تخصصی چشم پزشکی نور پیوست .
دکتر فدرا حاجی زاده (چشم پزشک) خدمات جراحی شبکیه، ویتره و لیزر، اصلاح عیوب انکساری( پی آر کی، لازک، فمتولیزیک، اسمایل)، کاتاراکت(آب مروارید)، تزریقات داخل چشمی، درمان بیماری های التهاب چشم(یوئیت) را در بیمارستان چشم پزشکی نور انجام می دهد.
